Job summary

Johns Hopkins Care at Home is seeking an LPN to provide skilled nursing care in patients' homes under RN supervision in the Baltimore area. The role includes patient teaching, care coordination, and scheduling visits to meet patient needs.

Ideal candidates have an LPN license, CPR certification, and a Maryland driving license. Prior home care experience is preferred; strong communication and advocacy for patients are essential.

• CPR - Basic CPR (Required)
• Prior work experience not required
• Valid Maryland Driver's License and current auto insurance. Must have working vehicle
• 2-3 years of clinical experience strongly (Preferred)
• Previous home care experience (Preferred)

The Home Health Services LPN provides, skilled nursing care for a group of patients usually in the setting of the patient’s home under the supervision of an RN. Employs the nursing process to perform skilled care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the LPN provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Licensed Practical Nurse ensures adequate service provision, including scheduling of visits, based on patients' identified needs and priorities for care under the direction of an RN. Complies with all Home Care policies and procedures.

Work Location: Anne Arundel County

Work Hours: must be available from 8:30a-5p. However, patient visit times are flexible.

Salary Range: Minimum 28.91/hour - Maximum 44.99/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
