Job summary

Ibis Healthcare is seeking a Care Manager for its Network Project and PHAME to provide advocacy, coordination, and continuity of evidence-based care for individuals and families through triage, assessment, planning, and communication. You will participate as a multidisciplinary team member and report to the unit supervisor.

Qualifications

• Bachelor's degree in a Human Services related discipline and at least 1 year experience in the mental health field.
• Ability to read, analyzes, and interprets clinical records, procedures, or governmental regulations.
• Ability to write reports and document clinical activities and effectively communicate with managers, patients, service providers, families, and other stakeholders.
• Must have a valid Florida driver's license, current vehicle insurance and reliable transportation.

Responsibilities

• Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
• Complete all clinical and administrative documentation based on established timelines as per program requirements.
• Communicate effectively and maintain cooperative relationships with internal and external resources.
• Develop service plans, treatment plans and/or aftercare plans with clear goals and objectives as per program requirements.
• Maintain efficient workflow management and organization as per program requirements.
• Maintain proficient quality of clinical documentation as per program requirements.
• Other duties as assigned or needed as per program requirements.
