Ibis Healthcare seeks a Care Manager for Central Intake to advocate, coordinate, and ensure continuity of evidence-based care within an Adult Inpatient Crisis Stabilization unit. You will participate in a multidisciplinary team and support triage, assessment, planning, and ongoing care services.

PRN staff are essential members scheduled as needed to facilitate triage, assessment, and care for individuals. Strong communication, documentation, and privacy practices are essential to success in this

Qualifications

• Bachelor's degree in a Human Services-related field and at least 1 year in the mental health field.
• Child welfare applicants require 2 years with at-risk populations and a child welfare certification.
• Ability to read, analyze, and interpret clinical records and regulations.
• Ability to write reports and document clinical activities; communicate with managers, patients, service providers, and other stakeholders.

Responsabilités

• Complete triage and psychosocial assessments and coordinate community resources.
• Complete clinical and administrative documentation within established timelines.
• Communicate effectively and maintain cooperative relationships with internal and external resources.
• Develop service, treatment, and/or aftercare plans with clear goals and objectives.
• Maintain efficient workflow and organize tasks as per program requirements.
• Maintain quality of clinical documentation per program requirements.
• Provide excellent customer service and servant leadership in all communications.
• Adhere to patient privacy and confidentiality practices.
• Assist staff with support in all areas of responsibility.
• Demonstrate empathy for patients and their disorders and effects on behavior.
• Possess physical agility to safely restrain patients during critical situations in collaboration with the team.
• Perform other duties as assigned.
