Job summary
La Frontera Arizona in Apache Junction is seeking a Case Manager to deliver coordinated behavioral health services to adults, children, and families across home, office, and community settings. You will support clients through crisis intervention, transportation, and education, aligned with the Arizona Practice Model and 12 Principles.
The role requires meeting BHT requirements or equivalent, maintaining complete documentation, and collaborating with stakeholders.
Qualifications
- Must meet BHT requirements as stated.
- Fingerprint clearance card or eligible to obtain one.
- Valid AZ driver's license with no major infractions in the past 3 years.
- Regular access to reliable transportation.
- Bi-lingual (Spanish).
- At least 21 years of age.
Responsibilities
- Provide direct in-home, in-office and community services including crisis intervention and transportation.
- Coordinate Child and Family Team Meetings and Adult Recovery Team Meetings; document and share outcomes.
- Maintain client records per RBHA, state and federal requirements; ensure thorough clinical documentation.
- Attend regular weekly supervision and meet productivity standards with defined direct client contact.
