Job summary

Child & Family Support Services, Inc. is seeking a Full-time Case Manager in Coconino County to coordinate behavioral health services for young adults and families in homes and communities.

This role is non-supervisory and involves extensive field work, scheduling flexibility, and collaboration with care teams. The position requires a BHT with two years of related experience or a related bachelor’s degree, 21+ years old, AZ driver license, insurance proof, drug test, fingerprint clearance, and

Qualifications

• Must be BHT with two years of related behavioral health work experience or related bachelor’s degree.
• Must be 21 years or older.
• Valid Arizona Driver's License with a 39 month driving record.
• Reliable transportation with proof of Insurance.
• Candidates must submit a negative drug test.
• Obtain an Arizona DPS Level One Fingerprint Clearance Card.
• Complete Adult, Child, and Infant CPR and First Aid training.

Responsibilities

• Provide ongoing case management; coordinate behavioral health services, medical care, education, placement, finances, and socialization services to children and families with intense/complex needs.
• Collaborate with families and team members to create support plans capturing strengths, needs and culture.
• Facilitate Child and Family Team meetings and maintain a strength-based environment.
• Assist families in securing needed community resources and linkages to other services.
• Engage in service delivery including skills training and family support.
• Interact professionally with stakeholders (DCS, DDD, juvenile probation, DSP, etc).
• Perform other duties related to the position.
