Qualifications

Responsibilities

• Provide case management services to clients.
• Collect data from collateral sources.
• Review records and other pertinent data and resources.
• Participate as a member of a treatment team in planning and reviewing services.
• Help identified children and families access medical, social, educational, vocational, and recreational services.
