Job summary

AWARE is seeking an SDMI Adult Case Manager to join our team in Polson, MT. This full-time role starts at $22.00 per hour and offers a path to professional growth within a supportive, inclusive work environment.

You will manage caseloads, coordinate services, develop Individual Treatment Plans, and collaborate with the SDMI Waiver team to connect clients with needed supports. We value compassionate care and community-focused outcomes.

Qualifications

• BS/BA in a social work-related field and minimum of one year direct work experience in adult mental health case management.

Responsibilities

• Meet monthly, quarterly and yearly caseload and billing expectations.
• Maintain caseload with Clinical Administrator and collaborate with SDMI Waiver teams.
• Assess adults to determine need for case management and SDMI Waiver services.
• Develop and monitor Individual Treatment Plans (ITP).
• Coordinate services and assist in accessing needed benefits and supports.
• Record notes and data in the EHR system.
• Participate in required trainings and team meetings.
