Job summary

The Phoenix Rehabilitation and Nursing Center is seeking an ADON / Risk Management Nurse (RN) to join our leadership team. This role oversees clinical operations and leads risk management, quality assurance, and regulatory initiatives to ensure resident safety and high-quality care.

Ideal candidates have an active RN license, prior skilled nursing experience, and a track record in leadership and compliance.

Qualifications

• Active and unencumbered RN license.
• Prior experience in skilled nursing or long-term care.
• Leadership experience (ADON, Unit Manager, or similar role preferred).
• Strong knowledge of regulatory compliance and risk management practices.
• Excellent communication, documentation, and organizational skills.
• Proven ability to lead teams and drive clinical outcomes.

Responsibilities

• Assist the Director of Nursing in daily clinical operations.
• Provide supervision, guidance, and support to nursing staff.
• Participate in staffing oversight and performance management.
• Promote clinical best practices and adherence to standards of care.
• Lead incident investigations and root cause analyses.
• Monitor and track adverse events, trends, and corrective actions.
• Ensure compliance with state and federal regulations.
• Support survey readiness and coordinate responses to audits.
• Collaborate with interdisciplinary teams to reduce liability exposure.
• Participate in QAPI initiatives.
• Analyze clinical data and implement performance improvement plans.
• Monitor infection control, falls prevention, wound care, and medication management programs.
