The Phoenix Rehabilitation and Nursing Center is seeking an ADON / Risk Management Nurse (RN) to join our leadership team in New York. This role supports the Director of Nursing in clinical operations while guiding risk management, quality assurance, and regulatory compliance to promote resident safety and excellence.

Ideal candidates will have an active RN license, leadership experience in skilled nursing, and a strong track record in improving clinical outcomes and reducing liability exposure.

Qualifications

• Active RN license in good standing.
• Experience in skilled nursing or long-term care preferred.
• Leadership experience in ADON/Unit Manager or similar role preferred.
• Strong regulatory compliance knowledge and risk management understanding.

Responsabilités

• Assist the Director of Nursing in daily clinical operations.
• Provide supervision and guidance to nursing staff.
• Oversee staffing oversight and performance management.
• Lead incident investigations, root cause analyses, and corrective actions.
• Ensure survey readiness and coordinate audits and compliance efforts.
• Collaborate with interdisciplinary teams to reduce liability exposure.
• Participate in QAPI initiatives and quality improvement programs.

Position Overview

The ADON / Risk Management Nurse (RN) supports the Director of Nursing in overseeing clinical operations while leading the facility’s risk management and quality assurance initiatives. This role plays a critical part in ensuring regulatory compliance, reducing risk exposure, and promoting resident safety and clinical excellence.

ADON / Risk Management Nurse (RN) Key Responsibilities

Clinical Leadership

• Assist the Director of Nursing in daily clinical operations

• Provide supervision, guidance, and support to nursing staff

• Participate in staffing oversight and performance management

• Promote clinical best practices and adherence to standards of care

Risk Management & Compliance

• Lead incident investigations and root cause analyses

• Monitor and track adverse events, trends, and corrective actions

• Ensure compliance with state and federal regulations

• Support survey readiness and coordinate responses to audits

• Collaborate with interdisciplinary teams to reduce liability exposure

Quality Assurance

• Participate in QAPI initiatives

• Analyze clinical data and implement performance improvement plans

• Monitor infection control, falls prevention, wound care, and medication management programs

ADON / Risk Management Nurse (RN) Qualifications

• Active and unencumbered RN license

• Prior experience in skilled nursing or long-term care

• Leadership experience (ADON, Unit Manager, or similar role preferred)

• Strong knowledge of regulatory compliance and risk management practices

• Excellent communication, documentation, and organizational skills

• Proven ability to lead teams and drive clinical outcomes

Salary: Up to $145,000 per year (Based on Experience)
