Job summary

CareCore at Mentor is seeking an experienced Registered Nurse (RN) to join our leadership team as the Assistant Director of Nursing (ADON) and Infection Preventionist. This role focuses on supporting clinical staff, upholding high standards for infection prevention, resident care, and regulatory compliance.

Key responsibilities include assisting the Director of Nursing with daily operations, serving as Infection Preventionist, monitoring infections, maintaining policies, and providing staff

Qualifications

• Active Ohio RN license required.
• Long-term care/ skilled nursing experience required.
• Infection prevention/control experience preferred.
• Nursing leadership or supervisory experience preferred.
• Strong knowledge of state and federal LTC regulations.
• PCC/PointClickCare experience preferred.
• Strong communication, organization, and leadership skills.
• Must be comfortable providing hands-on support when needed.

Responsibilities

• Assist the Director of Nursing with day-to-day clinical operations.
• Serve as Infection Preventionist for the facility.
• Monitor, track, and investigate infections and infection trends.
• Maintain infection prevention policies, procedures, and documentation.
• Provide infection control education and training to staff.
• Assist with survey readiness and regulatory compliance.
• Support nursing staff through education, coaching, and clinical oversight.
• Participate in audits, quality improvement initiatives, and resident care reviews.
• Assist with staffing, employee performance, and clinical concerns as needed.
• Provide leadership coverage and support to the DON and nursing department.
