Job summary

CareCore at Mentor is seeking an experienced RN to join our leadership team as Assistant Director of Nursing (ADON) / Infection Preventionist. This role combines hands-on clinical support with infection prevention leadership to uphold high resident care standards.

The successful candidate will assist the DON with daily operations, oversee infection prevention programs, mentor staff, ensure survey readiness, and drive quality improvement across the nursing department within a long-term care

Qualifications

• Active Ohio RN license required.
• Previous long-term care/skilled nursing experience required.
• Infection prevention/control experience strongly preferred.
• Previous nursing leadership or supervisory experience preferred.
• Strong knowledge of state and federal LTC regulations.
• PCC/PointClickCare experience preferred.
• Strong communication, organization, and leadership skills.
• Must be comfortable working collaboratively and providing hands-on support when needed.

Responsibilities

• Assist the Director of Nursing with day-to-day clinical operations.
• Serve as the facility’s Infection Preventionist.
• Monitor, track, and investigate infections and infection trends.
• Maintain infection prevention and control policies, procedures, and documentation.
• Provide infection control education and training to staff.
• Assist with survey readiness and regulatory compliance.
• Support nursing staff through education, coaching, and clinical oversight.
• Participate in audits, quality improvement initiatives, and resident care reviews.
• Assist with staffing, employee performance, and clinical concerns as needed.
• Provide leadership coverage and support to the DON and nursing department.
