HopeWest in Grand Junction, CO, seeks an Admissions Registered Nurse for full-time duties. The role coordinates referrals and admissions across home, hospital, and facility settings and serves as the main contact for discharge planners.

Responsibilities include informing patients/families about HopeWest services and helping select the appropriate program, with a focus on compassionate, patient-centered care. A sign-on bonus is available for eligible applicants.

Admissions (Access) RN Grand Junction Community Hospice Pay Range: ASN: $38.02–$41.75/hour BSN: $39.20–$43.05/hour $10,000 Sign-On Bonus 4-10's (8am-6pm), rotating days of the week to include some weekends (average of 2 every 6 week schedule)

SUMMARY OF JOB RESPONSIBILITIES:

The Admission RN is responsible for providing information about HopeWest services and community resources to individuals with serious illnesses. This role involves assisting patients and their families in determining which HopeWest service line best meets their needs and determining their eligibility for those services. As the primary point of contact for hospital discharge planners, the Admission RN coordinates and completes the referral and admission process in various settings, including home, hospital, and facility environments.

QUALIFICATIONS:

• Valid and current RN license in the state of Colorado maintained throughout employment.
• Graduated from an accredited school of nursing; Bachelor’s degree preferred.
• Hospice experience required
• Must have or obtain hospice certification (CHPN) by the end of the third year of employment.
• Valid Colorado driver’s license, unlimited access to reliable transportation, and adequate automobile insurance maintained throughout employment.
• Able to work in a variety of environments.

COMPETENCIES & REQUIREMENTS:

• Ability to set priorities and follow through.
• Demonstrates clinical excellence in assessing patient/family situation.
• Able to participate in the on-call, holiday, weekend, and after-hours programs.
• Current CPR certification is maintained throughout employment.
• Communicates effectively, both written and verbal, with a variety of customers
• Adequate computer skills preferably with Microsoft office products.
