Job summary

HopeWest is seeking a Registered Nurse (Variable) for palliative care in Meeker. You will deliver comprehensive nursing care, coordinate chaplains, social workers, and physicians, and support patients and families in meaningful ways.

The role requires at least 2 years of clinical nursing experience, a Colorado RN license, CPR, and a valid driver’s license. PRN shifts include weekend commitments and flexible scheduling.

Qualifications

• Education: Graduated from an accredited school of nursing; Bachelor’s degree preferred.
• Experience: Minimum of two years clinical nursing; hospice/palliative care or related settings preferred.
• Licenses: Colorado RN license, CPR, driver’s license, and auto insurance maintained throughout employment.

Responsibilities

• Provide comprehensive palliative nursing care through assessment, care planning, and symptom management.
• Serve as case manager for an assigned patient caseload and coordinate care across settings.
• Collaborate with physicians, community partners, and interdisciplinary team to support quality of life for patients with serious illness.

Job Summary

The Registered Nurse (Variable) provides comprehensive palliative nursing care through assessment, care planning, symptom management, patient and family education, care coordination, and interdisciplinary collaboration. This position serves as a case manager for an assigned patient caseload while ensuring compassionate, patient‑centered care that supports quality of life for individuals living with serious illness. The RN collaborates with physicians, community partners, and the interdisciplinary team to coordinate care across multiple settings while promoting HopeWest's mission and values. The registered nurse is responsible for providing patient care to assigned patients within the life span of all ages.

Availability

Must be available for at least 24 hours of work per month. They may identify the days and shifts they are available but must include 1 weekend shift. A general availability should be provided, with a more detailed availability submitted the month prior to scheduling. PRN's are expected to work up to the 24 hours per month, unless no shifts are available for the approved divisions they are trained or cleared for.

Minimum Qualifications

• Education: Graduated from an accredited school of nursing is required. Bachelor’s degree in nursing is preferred.
• Experience: Minimum of two (2) years of clinical nursing experience required; experience in hospice, palliative care, home health, case management, or acute care preferred.
• Licenses and Certifications: Current and valid Registered Nurse (RN) license in the state of Colorado, maintained throughout employment. Valid Colorado driver’s license, reliable transportation, and proof of required automobile insurance maintained throughout employment. Must possess and maintain current CPR certification throughout employment.

Knowledge, Skills And Abilities

• Knowledge of professional nursing practice, palliative care principles, symptom management, and applicable clinical standards.
• Strong clinical assessment, critical thinking, and sound judgment to identify patient needs and prioritize care.
• Ability to coordinate care effectively within an interdisciplinary team while managing multiple patients and competing priorities.
• Excellent verbal and written communication skills with the ability to educate patients, families, caregivers, and healthcare professionals.
• Ability to establish therapeutic relationships while demonstrating compassion, empathy, professionalism, and cultural sensitivity.
• Proficiency in electronic medical records (EMR), documentation standards, and Microsoft Office applications.
• Strong organizational, time management, and problem‑solving skills with the ability to work independently in a variety of care settings.
• Ability to maintain confidentiality, exercise professional discretion, and uphold HopeWest's mission, values, and ethical standards.

Pay & Benefits

• Pay: $35.02-$40.05 base wage, plus 10% for PRN status — (differentials: $5/hr weekends & $6/hr nights)
