Job summary

HopeWest in Grand Junction, CO, is seeking a Primary RN for Community Hospice Monday–Friday 8am–5pm. You will lead the interdisciplinary team, manage a patient caseload, and deliver relationship-based, compassionate care to patients in home settings.

The role requires 1–2 years of nursing experience, Colorado RN license, CPR, and comfort with EHR systems. Join a mission-driven nonprofit that supports caregivers and patients with autonomy and meaningful work.

Qualifications

• Minimum 1–2 years of nursing experience preferred.
• Current Colorado RN license maintained throughout employment.
• Current CPR certification maintained throughout employment.
• Experience in home health, hospice, or acute care strongly preferred.

Responsibilities

• Primary RN has an assigned caseload and leads the interdisciplinary team.
• Provide compassionate care, symptom management, and goal-based planning.
• Care for patients wherever they call home: private homes, nursing facilities, etc.
• Educate patients and caregivers to improve confidence in care.

Primary RN – Grand Junction Community Hospice | Monday–Friday 8am-5pm| $10,000 Sign-On Bonus

Schedule: Monday–Friday, 8:00am–5:00pm, with occasional on-call/weekend requirement

Location: Grand Junction, Colorado

Sign-On Bonus: $10,000

Pay Range

• ASN: $38.02–$41.75/hour
• BSN: $39.20–$43.05/hour

Primary RN’s have an assigned caseload of patients and lead the interdisciplinary team caring for those patients. We provide care wherever our patients call home - private homes, nursing homes, group homes or assisted living facilities.

What You Bring
