Job summary

Eagleville Hospital is seeking an admissions coordinator to review referrals, coordinate admissions, and liaise with Counseling, Nursing, Medical, and Psychiatric teams to ensure smooth transitions into the facility. Responsibilities include interviewing potential patients, managing referral tracking, conducting initial screenings, and securing timely funding

Qualifications

• Bachelor’s degree required.
• MSW or related field preferred.
• Experience with substance use disorders and mental health treatment is preferred.

Responsibilities

• Review referrals and facilitate admissions coordination with Counseling and/or Nursing departments.
• Conduct interviews of potential patients via phone or Zoom when appropriate.
• Maintain referrals tracking system for timely admissions.
• Provide initial screening and level of care assessments for admitted patients.
• Coordinate admissions and transfers with referral sources to optimize bed availability.
• Verify benefits and funding, including county funding where applicable, to ensure admission funding is active.
