Qualifications

• Bachelor's degree required.

• Experience in managed care pre-certification, level of care assessments and utilization preferred.
• Will require some travel to local community referral sources.

Responsibilities

• Facilitate review process of admissions and referrals.
• Use phone calls and Zoom for interviews of potential patients.
• Maintain tracking system of referrals.
• Provide initial screening and level of care assessment for patients calling to be admitted.
• Collaborate with Nursing, Medical or Psychiatric leaders for recommendations.
• Coordinate admission and transfer from referral sources to optimize bed census.
• Report a one week snapshot of anticipated admissions.
• Coordinate insurance verification and funding; verify patient benefits at admission; contact PA WITS for county funding if needed.
• Communicate projected admissions to internal representatives.
• Assist Nursing leaders to obtain clinical information from referral sources prior to admission.
• Serve as a community liaison and expand partnerships with referral sources.
• Maintain patient confidentiality and professionalism; address complaints; prioritize patient safety.
• Other related duties as assigned.

Job Details: Job Location: Eagleville Hospital - Eagleville, PA 19403, Position Summary

This position reports to Day Supervisor. Clinical professional responsible for reviewing referrals and facilitating the coordination of admissions, including but not limited to forensic referrals, maintaining relationships with county and state referral sources. Responsible for gathering necessary information and facilitating smooth transitions into the facility by coordinating with the Counseling and/or Nursing departments.

Objectives / Responsibilities

• Facilitate review process of potential admissions and referrals.
• Utilize phone calls and zoom for interviews of potential patients as appropriate.
• Maintain tracking system of referrals.
• Provide initial screening and level of care assessment for patients calling to be admitted.
• Collaborate with the Nursing, Medical or Psychiatric department leaders to ensure appropriate recommendations regarding pending referrals.
• Coordinate admission and transfer from referral sources. Strategically align admissions to optimize open beds in relation to census management.
• Responsible for reporting a one week snapshot perspective of anticipated admissions.
• Facilitate insurance verification process and secure appropriate funding. Perform benefit eligibility to ensure patient benefits are active at the time of admission. If needed, contact the Single County Authority to obtain county funding and completing necessary consents and paperwork in PA WITS.
• Collaborate with the Nursing, Medical or Psychiatric department leaders to ensure appropriate recommendations regarding pending referrals.
• Communicate projected admissions to designated internal representatives promptly.
• Assist the Nursing leaders with ensuring all clinical information from referral sources or patient (including medical comorbidity information) is received, when possible, prior to patient admission.
• Serve as a community liaison and maintain existing partnerships with referral sources within the community. Responsible for identifying and partnering with new perspective community partners.
• Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
• Respect patient confidentiality and privacy.
• Other related duties as assigned.

Qualifications: Educational Requirements

• A Bachelor’s degree required.
• A Social Work or Master’s degree in a social services field (psychology, counseling, social work, sociology, or health administration) from a college or university accredited by the Council on social Work Education preferred.

Qualifications
