Job summary

Eastern Health Group seeks a Hospital Liaison to lead and coordinate referrals across the assigned market. You will evaluate patient referrals and direct them to appropriate facilities, developing tailored solutions to meet identified needs and drive referrals.

Responsibilities include building partnerships with facility leadership, managing census goals, and maintaining strong communications with admissions teams.

Qualifications

• Current RN license in the state of practice preferred; LPN will be considered.
• Three or more years of experience in long-term care or related industry.
• One to two years sales and/or marketing experience a plus; case management experience preferred.

Responsibilities

• Meet physical and sensory requirements and work in the described environment.
• Identify and participate in process improvement initiatives to improve customer experience and workflow.
• Develop partnerships with Administrators and Admissions Director to set expectations.
• Build relationships with facility staff to identify and support needs.
• Generate referrals to facilities to support census and quality mix goals.
• Respond timely to inquiry calls from discharge planners and other referral sources.
