Job summary

Eagleville Hospital is seeking an Admission Referral Nurse to foster relationships, review potential admissions, coordinate admissions and transfers, and collaborate with the interdisciplinary team to optimize bed utilization.

The role supports thorough nursing assessments, effective hand-offs, and proactive communication to ensure timely and safe patient admissions within the behavioral health setting.

Qualifications

• Graduate of an accredited nursing program.
• Bachelor's degree in nursing preferred.
• Current PA State RN licensure in good standing.

Responsibilities

• Foster relationships and facilitate clinical review of admissions.
• Coordinate admission and transfer and bed coordination.
• Collaborate with all disciplines and assist with bed coordination.
• Provide comprehensive hand-off communication (SBAR) to receiving unit RN.
• Alert interdisciplinary team of changes in patient condition on arrival.
• Assist with admission assessments for direct admissions as assigned.
