Zusammenfassung

CommuniCare Health is seeking a full-time Admissions Coordinator in Fayetteville, WV to oversee admissions coordination and ensure all required paperwork is completed prior to admission. The role focuses on managing resources and delivering quality care in a long-term care setting.

The ideal candidate has at least 1 year of professional experience in long-term care, supervisory exposure, and solid knowledge of Medicare/Medicaid procedures.

Qualifikationen

• Minimum of 1 year of professional experience in a long-term care environment.
• Prior supervisory experience and management training.
• Knowledge of Medicare and Medicaid requirements and procedures.
• Ability to make independent decisions when circumstances warrant.
• Able to interact tactfully with residents, families, staff and external agencies.
• Ability to plan, organize, develop, implement, and interpret programs and procedures.

Aufgaben

• Give tours of the center.
• Handle daily admission calls to the center and follow up on inquiries.
• Follow up on referrals and admissions status.
• Review referrals and bed availability with the Review Team.
• Coordinate hospital/home visits and determine financial status (Private, Medicaid, Medicare).
• Notify staff of new admissions and perform census and discharge procedures.
• Maintain up-to-date knowledge of financial aspects for accurate reimbursement.
