Apploi is seeking an Admissions Coordinator/Liaison to join our nursing home team. You will coordinate the intake process, gather documentation, and ensure smooth admissions for patients and families.

In this role, you will liaise with patients, families, and healthcare providers, manage records transfers, and support regulatory compliance. Your communication and organizational strengths will drive a positive admission experience.

• Communicate with patients, families, and healthcare providers to gather information and provide updates on admission status
• Facilitate the transfer of patient records and information to our facility
• Develop and maintain relationships with healthcare providers and community partners to ensure timely and efficient admission processes
• Provide exceptional customer service, addressing patient and family concerns, and ensuring a positive experience during the admissions process
• Collaborate with the Admissions team to ensure compliance with regulatory requirements and organizational policies
• Participate in process improvements and quality initiatives to enhance the admissions experience
