Job summary

Sangre de Cristo Community Care is seeking an Admissions Coordinator to manage the referral process and ensure timely access to care for patients and families. This fast-paced role focuses on compassionate service, coordination with nurses, providers, and referral sources, and accurate documentation.

You will verify demographics, insurance, and orders, obtain authorizations when needed, and maintain HIPAA-compliant records while delivering outstanding customer care in a demanding environment.

Qualifications

  • High school diploma or equivalent
  • At least one year of healthcare intake, admissions, scheduling, or medical office experience
  • Typing speed of at least 34 WPM
  • Strong computer skills, including EMRs and Microsoft Office
  • Excellent organizational and communication skills

Responsibilities

  • Receive and process hospice, home health, and palliative care referrals by phone, fax, EMR, and secure email
  • Verify demographics, insurance, physician orders, and clinical information
  • Obtain prior authorizations when required
  • Explain services and obtain admissions paperwork and consents
  • Coordinate closely with nurses, providers, referral sources, and internal departments
  • Maintain accurate referral documentation and tracking logs
  • Prioritize urgent referrals while managing a high daily workload
  • Participate in daily team huddles and planning meetings
  • Protect patient confidentiality and maintain HIPAA compliance
  • Deliver exceptional customer service during what is often a stressful time for patients and families

What You'll Do
