Job summary

Avista Healthcare, Llc is seeking a compassionate Admissions Coordinator (CNA) to lead resident admissions in a community-focused facility. You will gather admission information, explain rights, and support families throughout the process, coordinating with nursing, business office, social services, and other teams to ensure a smooth experience.

The role emphasizes professional representation in the community, strong customer service, and empathy for residents and families, with a Monday–Friday

Qualifications

• Must be a Georgia Certified Nursing Assistant (CNA) in good standing.
• High school diploma or equivalent preferred.
• Excellent verbal and written communication skills.
• Strong computer skills and ability to learn facility software.

Responsibilities

• Obtain required information and signatures for resident admissions.
• Provide residents and families with admission information, including resident rights.
• Assist with interviewing residents, guardians, and sponsors and obtaining required admission information.
• Coordinate and complete admissions, transfers, and discharges per policies.
• Maintain accurate records of beds, census, room assignments, and admission activity.
• Review and explain admission-related information to residents/guardians and address questions.
• Maintain confidentiality of resident information per HIPAA and policies.
• Collaborate with nursing, business office, social services, MDS, and other departments.
