Job summary

National HealthCare Corporation seeks an Admissions Coordinator/Director to manage the admissions process, assess patient needs, and coordinate with social services, nursing, and bookkeeping to ensure smooth transitions into care. The role includes communicating with families, handling inquiries, and supporting crisis management within a hospital or center setting.

Daily responsibilities include collaborating with care teams, performing needs assessments, and maintaining community partnerships.

Qualifications

• Experience in a medical setting or knowledge of medical case management.
• Ability to work across multiple web-based platforms with strong computer skills.
• Knowledge of funding resources such as Medicare/Medicaid and managed care is highly preferred.

Responsibilities

• Coordinate admissions to the center by handling inquiries by phone or in person and conduct needs assessments.
• Support patients and families to adjust to center life and manage crises related to illness or disability.
• Participate in in-service training and maintain contacts with community agencies.
• Alert the Social Work Services Director or Administrator to identified admissions needs.
