Advanced-Health-Care-6 is seeking an RN Admissions Nurse to facilitate the admission of new residents and provide high-quality nursing care during transition. The role requires strong clinical skills, empathy, and excellent communication with residents and families.

The position is full-time with weekday and weekend coverage and an on-call rotation. You will collaborate with the care team, ensure regulatory compliance, and support discharge planning as needed.

Qualifications

• Valid RN or LPN license in the state of work.
• 1+ year of admissions or healthcare experience, preferably in skilled nursing/long-term care.
• Strong assessment and critical thinking with sound clinical judgment.
• Knowledge of admissions regulations and documentation for SNFs.
• Empathetic, patient-centered care with excellent communication skills.

Responsibilities

• Conduct thorough assessments of incoming residents to gather medical history and care needs.
• Collaborate with physicians and staff to develop individualized care plans.
• Explain admissions process and facility policies to residents and families.
• Complete admission paperwork and secure required documents.
• Coordinate with dietary and therapy for assessments and services.
• Administer medications as prescribed and monitor safety.
