Responsibilities

• Coordinate admissions and intake processes for new clients, ensuring a welcoming and supportive experience.
• Facilitate client screenings by phone and in-person to assess eligibility and service needs.
• Respond to referrals and document information in designated systems.
• Maintain program census and waiting lists in accordance with service standards.
• Collaborate with Care Managers to obtain and verify insurance information.
