The Skilled Nursing Admissions Coordinator is responsible for managing and coordinating the admissions process for prospective residents. This role involves working closely with hospitals, physicians, families, and internal teams to ensure a seamless and positive admission experience. The ideal candidate will have strong communication skills, a deep understanding of the long-term care industry, and a genuine passion for helping others.
Manage all aspects of the admissions process, including inquiries, tours, pre-admission assessments, and coordination of admissions paperwork
Collaborate with hospital discharge planners, case managers, and family members to assess referrals and determine appropriate placement.
Maintain accurate records in accordance with HIPAA and facility policies.
Ensure rooms are prepared and staff is informed of incoming residents.
Work with the interdisciplinary team to support resident care plans upon admission.
Market the facility’s services to referral sources and the community.
Provide tours and explain facility services to potential residents and families.
Track and report admissions metrics and maintain an active admissions pipeline.

High school diploma or equivalent required; Associate’s or Bachelor’s degree preferred.
Previous experience in admissions, case management, or marketing in a skilled nursing facility or healthcare setting strongly preferred.
Knowledge of Medicareand managed care insurance plans.
Excellent interpersonal, communication, and organizational skills.
Ability to handle sensitive situations with empathy and discretion.
Proficiency with EMR systems and Microsoft Office Suite.
