Admissions Coordinator
SUMMARY: The Admissions Coordinator is responsible for reviewing and preparing for new admissions in accordance with established policies and procedures. The incumbent will also maintain relations with community partners, family members, and personnel.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Assist in interviewing residents/guardians/sponsors and obtaining required admission information and signatures.
- Assist in the resident admission orientation program following established policies and procedures.
- Complete contracts for SNF, AL, & IL.
- Admit and prepare identification records for residents according to policies and procedures.
- Admit, transfer, and discharge residents in accordance with policies and procedures.
- Provide residents with admission information packets (e.g., resident rights, notice of privacy practices, admissions contracts).
- Maintain an accurate record of available beds and census.
- Ensure room readiness for new admissions.
- Abstract information from records as authorized/required for insurance purposes.
- Notify nursing staff when a resident has arrived to ensure escort to the assigned room.
- Perform administrative duties responsibly and accountably.
- Maintain confidentiality of all resident care information, including protected health information, and report any unauthorized disclosures.
- Follow up on referrals for AL/IL and assist with referral coverage when the Marketing Liaison is unavailable.
- Develop, schedule, and guide facility tours.
- Keep brochures and printed materials available for visitors.
- Maintain communication with families, residents, and community leaders.
- Plan and execute events to promote staff, resident, and community relations.
- Participate in community service and civic groups to promote charitable activities.
- Manage community service projects and develop problem-solving strategies.
- Attend workshops and seminars as approved.
- Perform other duties as assigned.
QUALIFICATIONS:
- High school diploma or GED required.
- Proficiency in Microsoft Office (Outlook, Word, Excel).
- Patience, tact, cheerful disposition, enthusiasm, and maturity to handle residents, staff, and visitors.
PHYSICAL DEMANDS:
- Ability to move intermittently throughout the workday.
- Ability to cope with mental and emotional stress.
- Ability to lift and move a minimum of 25 pounds to a height of 5 feet and carry such weight for at least 50 feet.
