Job summary

Infinity Healthcare Consulting is seeking an Admissions Concierge to manage admissions from pre-admission to discharge, ensuring smooth transitions and accurate contracts. You will conduct patient services rounds within the first 24 hours to gauge satisfaction and communicate needs to center leadership, supporting a positive patient experience.

The role requires mobility across health centers in the assigned territory and close collaboration with the Transition Advisor and care teams to deliver

Responsibilities

• Oversee all admissions from pre-admission to discharge, including admissions contracts, PASARRs and CHIRPs.
• Perform customer service rounds within 24 hours of admission to ensure patient satisfaction and report to leadership.
• Collaborate with Transition Advisor and center team to ensure seamless admissions and communicate patient needs and room readiness.
• Must mobilize between centers within assigned coverage territory.

The Admissions Concierge will be appointed to overseeing all admissions coming into the health center(s) from pre-admission to discharge, including admissions contracts, PASARR’s and CHIRP’s. As well, The Admissions Concierge will perform all customer service rounds within first 24 hours of patient stay ensuring patient satisfaction, reporting back to appropriate center leadership and ongoing throughout the duration of patient stay. Admissions Concierge will work closely with the Transition Advisor and health center team to create a seamless admissions experience including thoroughly communicating patient specific needs and room readiness. Must be able to mobilize from center to center within assigned coverage territory.
