Qualificações

• Bachelor's degree required in a clinically related field; Master's degree preferred.
• Current LPC, LISW, LISW-S or LSW; independent license preferred.
• 2-5 years recent clinical/administrative experience in behavioral health or medical settings.

Responsabilidades

• Facilitate patients through the health care system with case management for substance use and behavioral health needs.
• Educate providers, patients, and families about regulations, funding, resources and payors.
• Coordinate comprehensive assessment, treatment and follow-up evaluation with the care team.
• Assist in discharge planning and manage referrals, in-network providers, and appointments.
