The Care Coordinator assigned to this program facilitates patients through the health care system to include case management services for patients when presenting with substance use disorder and behavioral health conditions to the ED, hospital, or outpatient settings. The Care Coordinator also provides education & consultation to providers, patients, and families regarding the distinct & specific regulations, criteria, terminology, funding, resources, and third-party payors as they relate to addiction care.
