The Ohio State University Wexner Medical Center seeks a Care Coordinator for the System Wide Addiction Services to support patients through the health care system with case management and education.

With a caseload of 50+ adults, you coordinate assessment, treatment, and follow‑up, collaborate with clinicians and discharge planners, and educate providers and families about regulations and resources.

Qualifikationen

• Bachelor's degree in social work, public health, or related field.
• MSW preferred; independent license required.
• Licenses (LISW/LISW-S/LPC/LSW) or equivalent credentials required.
