The Ohio State University Wexner Medical Center is committed to enhancing the quality of care our patients receive. The Care Coordinator is a primary educator for the payers, providers, and the public. With a caseload of 50 or more adult patients, the Care Coordinator works with patients and referral staff to ensure stable linkages and minimize barriers to treatment in the community, including residential facilities, supportive housing, skilled nursing facilities, and home‑based placements. The Care Coordinator maintains an awareness of local and regional behavioral health treatment, criteria for admission and referral, and harm reduction services and state and national addiction care trends.
