Job summary

The Ohio State University Wexner Medical Center is seeking a Care Coordinator for the SWAS program to support patients with substance use and behavioral health needs across ED, inpatient and outpatient settings.

You will monitor patient care, collect data, collaborate with physicians, social workers and peers to coordinate assessments, treatment and follow-up, and educate payers and providers on regulations, funding and resources relevant to addiction care.

Qualifications

• Bachelor's degree in Social Work, Public Health, Allied Health or related field.
• Master's in Social Work preferred; independent license preferred.
• 2–5 years recent clinical or administrative experience in behavioral health or medical settings.
• Knowledge of multiple levels of care and local resources.

Responsibilities

• Coordinate comprehensive assessment, treatment and follow-up for patients.
• Provide case management and discharge planning with community linkages.
• Triaging referrals and locating in-network providers and resources.
• Educate payers, providers and the public on addiction care regulations and funding.
• Collaborate with physicians, social workers, peers and other health workers.
• Maintain awareness of local behavioral health trends and resources.
