UPMC seeks an Acute Service Coordinator I to provide mental health service coordination for chronically homeless individuals with serious mental health diagnoses, focusing on housing, service planning and referrals to promote stability in daily life.
The position is field-based, requiring a Bachelor’s in a Human Services field, with 1 year of related experience preferred. A $3,000 sign-on bonus is offered for an 18-month commitment.
The Acute Service Coordinator I provides mental health service coordination, according to the level of need, to a population of individuals considered chronically homeless with a serious mental health diagnosis and in need of housing. The primary function of Service Coordination is to provide professional assessment, assist with locating housing, service planning, service coordination, referral, and re-evaluation services required for a safe and healthy community life which is manifested through stability in relationships, housing, and employment. This is done in accordance with the Agency's vision, mission, and values, and emerging mental health Recovery Principles. This position provides an essential recovery orientation and is a field-based position.
This position is eligible for a sign-on bonus of $3,000 for an 18-month work commitment!
