In this role, you won’t just support individuals, you’ll be on the front lines of change, helping people rebuild their lives and rediscover hope.
At the heart of this work is a nationally recognized, evidence-based Housing First model that follows the approach of providing immediate, permanent, low barrier housing to people experiencing homelessness, with supportive services offered voluntarily. Safe, stable housing is the foundation for lasting wellness and meaningful recovery grounded in the powerful belief that housing is healthcare.
Through strong partnerships across the community, our Housing Case Management program is committed to ending homelessness in our county by providing housing navigation and placement support and ongoing, enhanced case management.
Working alongside UPMC Health Plan’s Community Team, housing providers, and community agencies and providers, you’ll support individuals facing housing instability, including those living on the streets, in shelters, couch surfing, or at risk of losing their housing. Our program also prioritizes those who are fleeing domestic or interpersonal violence and individuals whose health is directly impacted by their living conditions.
