Job summary

Qualifications

• Minimum of one year in an acute medical/surgical/ED or critical care nursing area.
• Associate Nursing degree required.
• Credentials from multiple states required, with state-specific licenses due post hire.

Responsibilities

• Oversee management and coordination of care for acute inpatient population.
• Participate in daily rounds and communicate discharge plans to the care team.
• Coordinate with social workers and case managers for post-acute care and transitions.
• Identify delays and barriers to discharge and provide corrective actions.
• Ensure follow-up appointments and referrals are scheduled prior to discharge.
• Adhere to safety, infection control, and regulatory requirements.
