Job summary

Responsibilities

• Conduct initial face-to-face assessment within 24 hours of admission to identify discharge needs.
• Document assessment outcomes in the electronic medical record and communicate discharge plans.
• Participate in daily rounds and monitor patient progression toward the discharge plan.
• Coordinate with social workers for long-term care planning and post-acute services.
• Identify delays in care, propose solutions, and escalate barriers as needed.
