Job summary

Kaiser Permanente in Virginia is seeking a Patient Care Coordinator to oversee care management for acute inpatient populations. Qualifications

• Minimum of one year in an acute medical/surgical/ED or critical care nursing area.
• Associate Nursing degree required.
• RN license in Virginia and MD/DC within 6 months or Compact RN license within 6 months.

Responsibilities

• Complete an initial face-to-face assessment within 24 hours of admission.
• Communicate assessment outcomes to the care team to inform discharge planning.
• Participate in daily Care Without Delay rounds and monitor progress toward discharge plan.
• Coordinate with social workers for long-term care and post-acute services.
• Ensure timely follow-up appointments and referrals prior to discharge.
