Job summary

Kaiser Permanente is seeking an Inpatient Care Manager (RN) to ensure appropriate admission levels, deliver cost-effective clinical coordination, and manage complex transitions across acute and post-acute settings. You will act as a clinical resource for physicians and teams, supporting discharge planning, community resource linkage, and system-wide care optimization.

The role requires a strong RN background with experience in inpatient settings and a proven ability to coordinate care

Qualifications

• Minimum of two years RN experience across Med/Surg, ICU, ED, Home Health, and related areas.
• Licensed as RN in WA or OR with ability to obtain other state licenses within 6 months of hire.
• Basic Life Support (BLS) certification required within 3 months of hire.
• Ability to work in interdisciplinary teams and advocate for patients throughout care transitions.
• Knowledge of discharge planning, transitional care processes, and utilization management.

Responsibilities

• Coordinates post-discharge patient care needs to assure timely and effective discharge from the hospital.
• Performs thorough patient assessments and documents care needs, health plan benefits, and level of care determinations.
• Collaborates with patients, families, and health-care team to develop plans of care optimizing resources.
• Facilitates safe transfers and coordinates with teams to ensure right care at the right time.
• Maintains ongoing contact with staff and community resources to ensure safe transitions.
• Ensures regulatory and compliance standards are met in collaboration with care teams.
• Performs duties as requested.
