Job summary

Kaiser Permanente is seeking a Patient Care Coordinator to oversee care management for acute inpatient populations. You will work with physicians, nursing, social workers, and ambulatory teams to ensure safe discharge and prevent readmissions.

The role requires strong communication, critical thinking, and time management skills; you will coordinate post-acute services and document progress daily within established policies.

Qualifications

• Experience using an electronic medical record system.
• Knowledge of health care reimbursement systems.
• Annual PCC Assessment of Critical Skills.
• Excellent communication with physicians, patients, and families.

Responsibilities

• Completes an initial face-to-face assessment within 24 hours of admission.
• Document assessment outcomes and determine discharge plan with care team.
• Participate in daily Care Without Delay rounds and monitor progress toward discharge.
• Document discharge plans per policies and regulatory requirements.
• Coordinate post-acute referrals and ensure timely follow-up for high-risk patients.
• Maintain infection control and patient safety policies and procedures.
