Responsibilities

• Oversee patient care management for acute inpatients and coordinate with physicians, nursing staff, and health team members.
• Conduct initial assessments within 24 hours of admission; develop discharge transition plans.
• Participate in daily rounds; document care progression, discharge plans, and barriers.
• Coordinate with social workers for long‑term care, financial assistance, and community services.
• Manage referrals for post‑acute care and escalate delays or barriers to care.
• Ensure follow-up appointments and referrals are scheduled before discharge.
• Adhere to safety policies and participate in quality assurance and professional practice.

Key Responsibilities Include

• Conducting initial assessments within 24 hours of admission to identify discharge needs and developing transition plans.
• Participating in daily rounds and documenting care progression, discharge plans, and barriers.
• Coordinating with social workers for long‑term care, financial assistance, and community services.
• Managing referrals for post-acute care and escalating delays or barriers to care.
• Ensuring follow-up appointments and referrals are scheduled before discharge.
• Adhering to safety policies, participating in quality assurance, and maintaining professionalism.
• Working collaboratively with patients, families, staff, and external providers to optimize patient outcomes.

Qualifications Include

• Minimum one year of experience in acute care nursing (medical, surgical, ED, or critical care).
• Associate Degree in Nursing required; BSN preferred.
• Registered Nurse license in applicable states or compact license within six months.
• Basic Life Support certification.
• Experience with electronic medical records, healthcare reimbursement systems, and strong communication skills.
• Ability to work independently with critical thinking, time management, and teamwork skills.

Additional preferred qualifications:
