Job summary

Kaiser Permanente in Maryland seeks a Patient Care Coordinator to oversee management and coordination of care for the acute inpatient population. The PCC collaborates with rounding MAPMG Hospital Based Service Physicians, patient/family, nursing, utilization review and other members of the healthcare team to assure continuum of patient care progression for clinical and cost-effective outcomes.

Qualifications

• One year in acute medical/surgical/ED or critical care nursing.
• Associate Nursing degree required.
• Credentials from multiple states; primary state at hire; additional post-hire.
• RN license (Virginia) within 6 months; RN license (Maryland) within 6 months; compact license; DC license within 6 months.
• Basic Life Support.

Responsibilities

• Completes an initial face-to-face assessment for every admitted member within 24hrs of admission.
• Document in KPHC and communicate assessment outcomes to determine transition plan with MAPMG team.
• Active participant in daily Care Without Delay rounds reporting progression toward discharge plan.
• Review and document discharge plan per policies and regulatory requirements.
• Document updates and barriers to discharge daily; escalate delays with solutions.
• Collaborate with Social Worker to coordinate long-term care, financial assistance, and other services.
• Send referrals and communicate with in-network vendors for post-acute care coordination.
• Ensure follow-up appointments and referrals to ambulatory case manager prior to discharge.
• Observe safety policies; participate in Quality Assurance activities.
• Maintain professionalism and work cooperatively with all stakeholders.
• Demonstrate excellent communication with physicians, members, families and staff.
