Job summary

Kaiser Permanente Towson, MD is seeking a Patient Care Coordinator to oversee care coordination for the acute inpatient population. You will collaborate with MAPMG Hospital Based Service Physicians, patient/family, nursing, and utilization review to ensure continuum of care and cost-effective outcomes.

You will work with community providers and ambulatory case managers to arrange appropriate level and transition of care for safe discharge and to prevent readmission.

Qualifications

• Minimum of one year in an acute medical/surgical/ED or critical care nursing area.
• Associate Nursing degree required.
• RN license in Virginia, Maryland, DC with 6 months to obtain compact license.

Responsibilities

• Oversee management and coordination of care for the acute inpatient population.
• Document discharge plans and communicate outcomes with care team.
• Participate in daily rounds and QA activities.
• Coordinate with social workers for long-term care and financial assistance.
• Ensure timely discharge and prevent readmissions.
