Job summary

ChenMed seeks an Acute Care Nurse for inpatient coordination and safe, timely discharge planning. You will advocate for patients and families, collaborate with hospital staff, PCPs, and care teams, and manage care plans across the care continuum.

You will document updates in the hospital EMR, participate in post-acute transitions, and help optimize resource use while focusing on high-quality patient outcomes.

Qualifications

• Bachelor’s Degree in nursing (BSN) or RN with bachelor’s degree preferred.
• Active RN license required in state of employment.
• Compac t license preferred where available.

Responsibilities

• Provide daily presence at assigned hospitals during core hours.
• Coordinate discharge planning and care across acute and post-acute settings.
• Communicate with family, PCPs, hospital staff, and care teams.
• Lead care management/disease management program discussions with patients.
• Document patient progress and discharge plans in the EMR system.
