Qualifications

• Associate degree in nursing is required.

Responsibilities

• Coordinate integration of social services and case management into patient care, discharge, and home planning.
• Conduct concurrent medical record reviews using approved indicators.
• Act as patient advocate; report adverse events; educate staff on resource utilization.
• Promote efficient use of clinical resources and meet defined outcomes within timeframes.
• Review admissions through discharge to evaluate service utilization and patient satisfaction.
• Communicate with physicians regularly to support appropriate cost, case and outcomes.
• Coordinate social services to help patients and families cope with illness impact.
