Job summary

ChenMed is seeking an Acute Care Manager, Complex Care (RN) to lead coordinated care across hospital and post-acute settings in Chicago. You will advocate for patients and families, coordinate with hospital teams, PCPs, SNFs, and health plans to ensure timely discharge and appropriate level of care.

This role requires strong interpersonal skills, clinical experience in case management, and proficiency with EMR, with travel up to 10% and BLS certification within 90 days.

Qualifications

• RN with BSN or related degree preferred.
• Active RN license required.
• BLS certification within first 90 days.
• Minimum 2 years clinical experience.
• Minimum 1 year of utilization review/case management.
• Willingness to travel up to 10%.
• CCMS certification preferred.

Responsibilities

• Lead acute and post-acute care coordination at hospitals.
• Discharge planning and transitions to SNF/LTC.
• Coordinate with hospital teams and family for care plans.
• Document patient progress in EMR daily.
