Job summary

ChenMed LLC in the United States seeks an Acute Care Manager, Complex Care (RN) to lead care coordination for seniors, oversee discharge planning, and ensure safe, timely transitions across inpatient, SNF, and home settings.

You will partner with hospital teams, PCPs, social workers, and family caregivers to develop and monitor individualized care plans, perform SDoH screening, and drive high-quality outcomes.

Qualifications

• Bachelor's degree in nursing (BSN) or RN with related field.
• Active RN license in state of employment.
• BLS certification within 90 days of employment.

Responsibilities

• Oversee hospital bedside care management and discharge planning.
• Coordinate transitions to SNF or home with PCPs and care teams.
• Report daily inpatient updates using hospital EMR.
• Perform SDoH screening and engage social workers as needed.
• Communicate discharge plans to patients, families, and providers.
