Job summary

Sierra Medical Center, part of Northern Nevada Health System, is seeking a per diem Physical Therapist to evaluate, plan, and administer therapy for adolescent to geriatric patients. The role requires collaboration with a high-functioning team and contributes to education of staff and program development.

Responsibilities include developing treatment plans, documenting progress, and coordinating patient care to ensure timely discharges.

Qualifications

• Current state Physical Therapy licensure required.
• Acute care experience required; hospital setting.
• New grads welcome if training in acute care is provided.
• Current BLS certification required.

Responsibilities

• Evaluate patients and develop treatment plans and goals; monitor progress.
• Report progress to staff, patients, and family; participate in rounds.
• Maintain all required documentation and discharge reports.
• Collaborate with multidisciplinary team; supervise staff or students as needed.
• Schedule and coordinate patient care; facilitate safe discharge and continuity of care.
• Participate in staff education and program development.
• Perform other duties as assigned.
