Job summary

Advocate Health seeks a licensed Occupational Therapist for part-time rehabilitation services at Sherman Hospital's Acute Care Rehabilitation unit in Illinois. • Master’s degree in Occupational Therapy required.
• Licensed Occupational Therapist in the state.
• Experience in acute care preferred but not required.

• Implement individualized therapy plans and progress notes.
• Discharge planning and home program education.
• Document according to policy and coordinate with care team.
