Qualifications

Responsibilities

• Interprets physician referrals and assesses patients’ physical, perceptual, cognitive, social, and vocational abilities and how they affect daily living.
• Develops and implements a plan of care; provides/ fabricates devices to improve independence; adjusts plans based on progress and physician input.
• Completes documentation, discharge planning, and patient education; coordinates with team and ensures billing and productivity standards.
